In order to make sure your visit to the practice goes smoothly, we've put together all the important information about how things work at the practice.
Don't forget to bring your current information with you
In order to register and treat you promptly, we request that our patients bring their current insurance information, your referral information, identification, and any custody papers available with them for each visit. If you have any questions on information needed at your visit please feel free to call us with any questions. We will review all of your information with you at each visit. We do collect all copays and other payments at time of service.
You can also find examples of our patient forms on our information for patient's page if you prefer to fill these out prior to your visit.
Be sure to check with your insurance or PCP to see if you will need a referral for your visit with Dr. Crnic. If a referral is required and we have not received it, you will be rescheduled for another day and time.

Please bring all paperwork that needs to be completed from your school, doctor, or other facility at the time of the appointment. Any forms that need to be completed will be done so and forwarded within 48 hours. Certain forms may require special permisson, any questions may be directed to the front desk.
For our patient's safety, proof of custody paperwork may be needed to confirm that the appropriate person is with us to care for your child. You can also assign designees to bring your child to appointments on our patient information form.
Your time is very important to us, please let us know within 24 hours if you are going to be unable to keep your appointment so we can allow all our slots to be used as efficiently as possible. A no show fee may apply if we are unable to contact you or don't hear from you. This is a courtesy to all our patients that need to be seen in a timely fashion. We will attempt to call and confirm your appointment with you so it is important to keep us updated with all your contact information each visit.
If the patient has seen an optometrist or another Ophthalmologist other than Dr. Crnic please be sure that we have medical records on your past history. It may be necessary to have a request for records form faxed to them, if so please let us know ahead of time so we can help you with that.
Please bring with you to your appointment proof of identification and residence, any insurance information you have, any custody or medical power of attorney paperwork, or permission forms needed to see the patient.
If the patient wears glasses please bring them to the appointment.
A current list of medication is necessary as you will need it to fill out the paperwork.

Monday and Wednesday 12 pm to 7 pm.
Tuesday and Thursday 9 am to 5 pm.
First and Fifth Friday 9 to 12 pm
Our office closes for inclement weather when Amarillo ISD closes.
